Tameside updates access to adult social care with launch of Early Support and Advice Hub

Residents who need support from adult social care are now being asked to contact Tameside Council’s Early Support and Advice Hub (ESAH) as their first point of contact.

Previously, calls for adult social care support were handled by the Community Gateway, but from Monday, March 30, anyone seeking help will need to contact ESAH directly on 0161 342 5656.

The Community Gateway will continue to operate in its role for Health District Nursing and can be contacted on 0161 922 4888. Find more information about the Community Gateway on the Trust website.

Residents who need support from adult social care are now being asked to contact Tameside Council’s Early Support and Advice Hub (ESAH) as their first point of contact.

The ESAH team take a holistic approach - looking at what matters to each person, helping people navigate their options and find the right care and support at the right time. Their primary aim is to support residents to stay well, independent, and connected to the place they call home and doing the things that matter most to them.

In the past year, the team have supported almost 5000 residents to live well in the place they call home.

Tameside Council Executive Member for Adult Social Care Cllr John Taylor, said: ‘’The team are dedicated and focused on making sure support fits each person’s unique needs. They look at all avenues in the community, shorter-term and equipment to keep them living as independently and well as possible in the place they call home.

“Please make sure to share this change with others, so people know where to go for support.’’

A Tameside and Glossop IC NHS FT spokesperson said: “The Community Gateway will continue to play an essential role as the single point of contact for District Nursing, offering a responsive, compassionate service for patients, carers and professionals.

“We will continue to work closely with Tameside Council to ensure members of our community can access the support they need as smoothly as possible.”
